Team Captain Certification and Waiver

By submitting this registration, I certify that I am the designated Team Captain and authorized representative of my team. I acknowledge and agree that:

• I have informed all players on my roster of the tournament rules, policies, and requirements.
• I understand that participation in flag football and tournament activities involves inherent risks of injury, illness, disability, property damage, and death.
• On behalf of myself and my team, I agree to abide by all rules, policies, and decisions of Northeast Flag Football Association, LLC.
• I understand that each player is responsible for determining their own fitness to participate and assumes all risks associated with participation.
• I release and hold harmless Northeast Flag Football Association, LLC, its owners, officers, members, employees, volunteers, sponsors, venue operators, officials, and affiliates from claims arising from our participation to the fullest extent permitted by law.
• I grant Northeast Flag Football Association, LLC permission to photograph, record, livestream, and use team and player images for promotional and media purposes.
• I understand that Northeast Flag Football Association, LLC reserves the right to remove any player, coach, or team for unsafe, abusive, or unsportsmanlike conduct without refund.
• I understand that schedules, formats, divisions, locations, and tournament operations may be modified as necessary due to weather, field conditions, safety concerns, or circumstances beyond the control of Northeast Flag Football Association, LLC.
